Will Insurance Cover My Medical Wig?

If you loose your hair due to a medical reason such as chemotherapy, alopecia or trichotillomania (hair-pulling-disorder), you may be able to file an insurance claim for a wig.*

Ask your insurance provider how much they are willing to reimburse you, and how long it will take to get reimbursed. Contact us for a custom quote if your insurance providers requests one.

Filing an Insurance Claim for your Wig

Your prescription should be written for a medical hair prosthesis/cranial prosthesis. In most cases, you can file a claim only after you purchase a medical wig. Claim Form #1500 (Sample Form) is the most commonly-used form, which you can get from your medical provider. You will need the following information to complete the form:

  1. Prescription from your doctor. It must be written for a medical or cranial hair prosthesis, and contain your diagnosis code

  2. Procedure Code from your insurance provider. The most common code is L8499 for Human hair wigs

  3. You will need our Federal Tax ID: Contact porsche@porschewaldo.com

  4. Receipt for the wig paid in full

What If I Don’t Have Insurance?

No worries! A wig can be deducted as a medical expense on your taxes. The cost of a wig is considered a medical expense if your doctor recommends the wig AFTER you experienced hair loss due to chemotherapy, alopecia, or any other medical condition. Please keep in mind, cosmetic wigs are NOT deductible. You will need the following information to file a claim:

  1. An itemized receipt for the wig (image of the products, date of purchase, cost, sales tax)

  2. Prescription from your physician for a medical or cranial hair prosthesis (not a wig)
